Vagus nerve stimulation safety: who should not use these devices
Consumer vagus nerve stimulation devices are generally well tolerated, and the most common side effect in published research is skin irritation where the electrode sits. You should not use one if you have an implanted electronic device such as a pacemaker, and you should talk to your clinician first if you are pregnant, have a heart condition, or have epilepsy. The manufacturers do not all publish the same warnings, which is the part most buyers miss.

The contraindication lists are not the same across brands
Most articles about vagus nerve stimulation safety talk about the category. That hides something practical: the two most widely sold devices publish different warnings, because they stimulate different places on the body.
We went to the primary documents. The Pulsetto list below is quoted from the current Pulsetto instruction manuals, and the Nurosym list from the manufacturer's published contraindications page.
Pulsetto, worn on the neck
From the Pulsetto Lite and Pulsetto FIT instruction manuals, section "When Not To Use Pulsetto". Avoid using it if:
- You have an implanted electronic device such as a pacemaker or hearing implant
- You have metal implants in the neck area
- The skin on your neck is broken, irritated, or has a rash
- You are in or near water, for example a shower, bath, or pool
Their separate safety notes add:
- Keep the device out of reach of children
- Do not use it while driving, operating machinery, or doing anything that needs your full attention
- Always apply the conductive gel as instructed
- End the session if you notice dizziness or light headedness
Nurosym, sold as Nuropod in the US, clipped to the ear
Not recommended if you:
- You have had a cervical vagotomy, meaning surgery that cut the vagus nerve
- You have been diagnosed with severe bradycardia, a very low resting heart rate
- You have a permanent metallic or electronic implant, or jewellery, close to the ear tragus that cannot be removed
- You are pregnant, which the manufacturer notes has not been evaluated in its clinical studies
Requiring medical supervision rather than ruled out entirely:
- An active implanted device, including a cochlear implant, cerebral shunt, invasive vagus nerve stimulator, or pacemaker
- Known severe coronary disease, or a heart attack within the past five years
- Children
The epilepsy and pregnancy question
This is the most searched safety question about these devices, and it deserves a straight answer rather than a hedge.
Pregnancy. Nurosym states plainly that pregnant women have not been evaluated in its clinical studies and is not recommended. Pulsetto's current manuals do not mention pregnancy at all. The absence of a warning is not the same as evidence of safety. It means the question has not been studied in the population you are asking about, so this is a conversation to have with your clinician rather than one to resolve from a product page.
Epilepsy. Implanted vagus nerve stimulation is an established, regulator-approved treatment for drug resistant epilepsy, which is why people reasonably assume the consumer versions are relevant. Pulsetto's current manuals do not list epilepsy as a reason to avoid the device. That silence is not an endorsement either. A consumer wellness device is not the implanted therapy, the two are not interchangeable, and anyone with a seizure disorder should raise it with the clinician managing that condition before using any stimulation device.
Pacemakers and implanted electronics. Here both manufacturers agree, and this is the one clear rule on the page. Pulsetto says avoid. Nurosym says medical supervision. Treat an implanted electrical device as a stop sign until your clinician says otherwise.
What the published research actually reports
Two systematic reviews have looked specifically at whether transcutaneous vagus nerve stimulation causes harm.
Brain Stimulation, 2018. A systematic review of 51 studies covering 1,322 people who received transcutaneous vagus nerve stimulation. The most common side effects were:

2.6% of participants, 35 people, left their study because of side effects. Thirty serious adverse events occurred across the whole set, and researchers judged only three of them as possibly caused by the stimulation. The authors concluded it is safe and well tolerated at the doses tested in research.
Scientific Reports, 2022. A larger systematic review and meta-analysis, 177 studies and 6,322 people, focused on ear stimulation. It found no difference in the risk of an adverse event between people receiving real stimulation and people receiving a sham, an overall incidence of 12.84 events per 100,000 person-minutes-days of stimulation, and no causal relationship between the stimulation and severe adverse events. The most frequently reported effects were ear pain, headache, and tingling.
One caveat worth stating, because nobody else does: several authors of the 2022 review disclose consulting relationships with a company developing this type of device. The finding is consistent with the earlier independent review, so it does not overturn the picture. It is still context you are entitled to have.
How to read both. Skin irritation is common and expected. Serious events are rare and mostly not attributable to the stimulation. That is a reassuring profile, and it comes with a real limit: these are supervised research settings, mostly short study periods, and mostly healthy or single-condition participants. It is not a statement about long term daily use at home.
What is normal to feel, and what is not
Pulsetto's own manual describes the expected sensations: mild tingling, warmth, or light muscle twitches where the device touches, and temporary skin redness. It states these are normally short lived and fade after the session.
The line the manufacturer draws, and it is a sensible one: if discomfort persists, stop using the device. Their safety notes also say to end a session if you feel dizzy or light headed.
Reasons to stop and check with a clinician rather than push through:
- Discomfort that does not fade after the session ends
- Dizziness, light headedness, or feeling faint
- Any change in heart rhythm you can feel
- Skin that stays irritated between sessions
Public discussion of these devices includes people whose nervous systems reacted badly to stimulation that most users tolerate easily. Sensitivity varies. Starting at the lowest intensity and short sessions is the low cost way to find out where you sit.
The category caveat that matters more than any warning list
These are general wellness products, not medical devices in the regulatory sense. Pulsetto's manual says so directly: it is not intended to diagnose, treat, cure, or prevent any disease. Nurosym carries CE medical device certification in Europe, which is a higher regulatory bar, though certification describes a manufacturing and conformity standard rather than a promise of a result.
Practically, that means two things. Nobody is monitoring your use. And if you are managing a diagnosed condition, a wellness device is an addition to care, never a replacement for it.
